Our Process - All Abilities Allied Health

Supporting you each step along the way

1. Initial Contact Reach out to us via phone, email, or our online form.

 Our friendly team will gladly introduce our services and guide you through our allied health treatment options.

2. We will call and book your Initial Consultation

We'll contact you at a convenient time to discuss different allied health services (Exercise Physiology and Occupational Therapy) and book your Initial Consultation (home visit).

3. Be heard during your Initial Consultation

During the Initial Consultation home visit, we'll listen to your story and ask questions to get to know you. Our Allied Health Practitioners utilise the Person-Centred approach and are trained to complete clinical Outcome Measures to best understand your current ability. 


These Outcome Measures will help setting individualised goals and develop a Action Plan together. 


4. Allied Health Intervention and Treatment Plan

Based on the initial assessment, you'll begin your personalised Action Plan. Our allied health practitioners guide and facilitate you in achieving your goals.


This guidance may include home visits, exercise or activity programs, expert advice, support with assistive technology (AT)and functional capacity assessments (FCA). 

 

5. Review and Follow-Up

We'll conduct regular reviews to track your progress and modify your care plan as necessary. Our objective is to keep you on the path to independence and achieving your goals. 


Some allied health treatment plans and referrals require specific Report Writing, including exercise physiology and occupational therapy services. Our practitioners are experienced in Report Writing to effectively advocate for you and communicate with your healthcare team including General Practitioner (GP), specialist, Support Coordinator (SC - NDIS), Plan Manager (NDIS), Case Manager (Lifetime Care or iCare).
